Access to Your Health Care Records
Consumers have the right to request copies of their health care records. To request your records:
Submit a written request to our office by emailing [email protected]
Include your full name, date of birth, and a description of the records requested.
Requests will be processed in accordance with Texas law.

Consumers may file a complaint with the Office of the Texas Attorney General – Consumer Protection Division if they believe their rights have been violated.
